Long-term Care Continuum
Long-term care (LTC) spans across a continuum of services and settings designed to support individuals, primarily seniors, who require health and personal care services over an extended period. 1
LTC services can be delivered at home, in hospital, or in a long-term care home, such as a nursing or residential care home. The LTC Continuum dashboard, currently focused on services provided in LTC homes, provides insight into how workforce availability aligns with resident needs and facility capacity by connecting staffing data with resident clinical profiles and quality-of-care indicators across provinces and territories.
